●Basic data: 77-year-old man
●Past history: diabetes mellitus and
arrhythmia
●Chief complaint: Acute onset of
slurred speech and left limbs weakness noted at 00:00 today
●Present illness:
○The patient was fully independent
in activities of daily living. His previous modified Rankin Scale (mRS) score was 1.
He went to bed at 8:00 PM last night in his usual state of health. At around
00:00, he repeatedly pushed his wife with his arm, prompting her to realize
that he had slurred speech
and weakness in his left limbs.
○Neurological Examination: GCS :
E4V5M6. Bilateral no hemianopia. Pupils: 2.0+/2.0+cm. Hemineglect to 2 different sensory
modalities (tactile, visual). EOM showed prefer rightward gaze,
leftward grossly full. No obvious facial palsy. Face sensory was symmetric.
Muscle power revealed upper 5/3- lower 5/4.
Finger-Nose-Finger and Heel-Knee-Shin showed no dysmetria. Sensory examination
was bilaterally symmetric to pinprick. Babinski
sign were both no plantar flexor. The
National Institute of Health Stroke Scale (NIHSS)
score was 8
